TO APPLY YOU MUST BRING WITH YOU:

Picture I.D. for each applicant and all adults in the household
Birth Certificate for all Children 12 and Under
(CHILD MUST BE 12 ON OR BEFORE DISTRIBUTION DAY)
Guardians must prove custody of any children, grandchildren and/or any minors living with you.

YOU MUST ALSO BRING PROOF OF YOUR EXPENSES:

Rent receipt or mortgage payment
Light Bill
Gas Bill
Telephone/Cell Phone Bill
Cable Bill
Car Payment
Any other expenses you have

DON’T FORGET PROOF OF YOUR INCOME:

Recent pay stub
Social Security Check/Letter
Child Support
Retirement Pension
Any other income you have

Wednesday October 28th from 9:00 AM - 3:00 PM..... Thursday October 29th from 9:00 AM - 6:00 PM..... and Friday October 30th from 9:00 AM - 6:00 PM.... At The Salvation Army Administrative Office 1824 W 15th Street, Panama City FL (Next to the Thrift Store) DUE TO THE LARGE NUMBER OF FAMILIES APPLYINGTHIS YEAR, THE APPLICATIONS' PROCESS WILL BE CLOSED ON FRIDAY, OCTOBER 30, AT 6 PM. PLEASE CONTACT OTHER AGENCIES IN TOWN FOR ASSISTANCE.
